It is helpful for us to gather information on your medical and dental background before you come in to see us. Also, by having the ability to fill out these forms online ahead of your scheduled appointment, it streamlines and shortens your visit with us here in the office. It will not be necessary for you to arrive early to your appointment in order to fill out paperwork - it will already be done! Patient AppointmentsPatients are seen by appointment only. We do not accept walk-in appointments. If you cannot keep your appointment, we ask that you notify us as soon as possible. A minimum of 48 hours advance notice is required in order to avoid being charged for a missed appointment. We do our best to schedule appointments at your convenience. Keeping your scheduled appointments helps us to better serve you and our other patients. Financial OptionsWe don't ever want finances to be a reason you don't visit the dentist. At David R. Moyer, DDS, we provide affordable plans to suit your personal budget. If you have dental or medical insurance, we will ask to see your insurance card ahead of time so that we can review your plan and then let you know what your insurance policy will cover. In order to best assist you, we ask that you submit your dental insurance information to us at the same time that you fill out your new patient forms online, at least two business days before your scheduled appointment. This allows us the time needed to contact your insurance company and gather the information about your plan so we can find out what your plan covers before your visit. At your visit we will be able to tell you what your insurance covers. This includes if any additional treatments are recommended for you. We will first give you an estimate of how much your insurance company will cover for any given procedure, and what portion will be your responsibility. We will never perform any dental work without first letting you know what is expected. In addition to accepting various forms of dental insurance, we have other financial options available for your visit as well. We accept payments via credit card, such as VISA, MasterCard, and American Express. We also accept checks and cash as acceptable forms of payment.
