Dr. Moyer and his dental team at David R. Moyer, DDS are here to help you every step of the way, from scheduling your first appointment and filling out necessary paperwork, to answering any billing or insurance questions you might have.
We want every part of your visit with us to be clearly understood, and a pleasurable experience.
New Patient Forms
Our website provides a space for you to access helpful new patient links such as new patient forms. It is helpful for us to gather information on your medical and dental background before you come in to see us. Also, by having the ability to fill out these forms online ahead of your scheduled appointment, it streamlines and shortens your visit with us here in the office. It will not be necessary for you to arrive early to your appointment in order to fill out paperwork - it will already be done!
Patients are seen by appointment only. We do not accept walk-in appointments. If you cannot keep your appointment, we ask that you notify us as soon as possible. A minimum of 48 hours advance notice is required in order to avoid being charged for a missed appointment. We do our best to schedule appointments at your convenience. Keeping your scheduled appointments helps us to better serve you and our other patients.
We don't ever want finances to be a reason you don't visit the dentist. At David R. Moyer, DDS, we provide affordable plans to suit your personal budget. If you have dental or medical insurance, we will ask to see your insurance card ahead of time so that we can review your plan and then let you know what your insurance policy will cover.
In order to best assist you, we ask that you submit your dental insurance information to us at the same time that you fill out your new patient forms online, at least two business days before your scheduled appointment. This allows us the time needed to contact your insurance company and gather the information about your plan so we can find out what your plan covers before your visit.
At your visit we will be able to tell you what your insurance covers. This includes if any additional treatments are recommended for you. We will first give you an estimate of how much your insurance company will cover for any given procedure, and what portion will be your responsibility. We will never perform any dental work without first letting you know what is expected.
In addition to accepting various forms of dental insurance, we have other financial options available for your visit as well. We accept payments via credit card, such as VISA, MasterCard, and American Express. We also accept checks and cash as acceptable forms of payment. We work with financing companies that have payment options as well. Dr. Moyer is a Delta Dental Premier Provider.
Most of the patients that come see us at David R. Moyer, DDS came to us because they have been personally recommended to us by another satisfied patient, not just because they randomly chose us from a list of dentists they weren't familiar with. Word of mouth is a great marketing tool.
We know recommendations and reviews are important to our patients. This is why we believe in the importance of posting client reviews and testimonial videos on our website so that you yourself can hear from others about their experience at our dental office.
We believe in continuing education for ourselves and our patients. That is why Dr. Moyer and his team offer regular educational blogs as a resource for our patients and the community.

We want to post and discuss your questions. If you have a question or would like a certain topic covered, feel free to contact us at our office. We will do our best to get this requested information up on our blog. Chances are, if you have a question, so does someone else!
We know that coming in to see the dentist involves more than just a teeth cleaning. While regular dental exams and cleanings are very important, oral hygiene and dental care goes deeper. Patients have questions about how to properly care for their teeth and gums. Some worry they are developing periodontal disease and may not know what to do about it.
What if you have missing teeth and aren’t sure if you should get a crown, a bridge, or dental implants? What about dentures? At David R. Moyer, DDS, we believe in being here for you and educating you as much as possible about your dental health. We want our patients to always be comfortable to ask questions, even the ones that may seem silly or awkward.
